In lieu of an abstract, here is a brief excerpt of the content:Francis Sparshott THE CASE OF THE UNRELIABLE AUTHOR Narratology, as the study of narrative in its most general sense, has made great advances in the last decade, ranging from the rhetorical and syntactic study of narrative forms to an interpretation of human life as a fabric of stories we tell ourselves and each other. I do not keep up with these studies, and the intention of die present (...) article is not to add to them. The standpoint from which it is written is not that of the literary theorist, but that of the relatively plain reader. I write as someone who reads stories that people have written for relatively naive people like me to read, though I have fortified myselfwith a handful of appropriately down-to-earth theoretical works to help me out. The article presents some reflections prompted by a novel that I have found disturbing, not as a special sort of being called a reader and correlated with an equally special writer, but as someone who has been reading a book and has had to come to terms with the fact of being transformed into a person who has read it. In 1966, the English novelist Anthony Burgess announced his intention of writing a novel "of Tolstoyan proportions."1 In 1980 it appeared, under the title Earthly Powers, and was duly described by some reviewers as his War and Peace.2 But why? Perhaps it was just the size of it, 649 pages. Perhaps it was the way the novel combined a story of private life with a background of world history. But perhaps what the reviewers found relevant in Tolstoy was not only the length and the epic scale but also the way his novel conveyed a view oflife, partly in the story invented, partly in the opinions of favorably presented characters, and partly in the way he supplemented his story with comments directiy addressed to the reader — or should we rather say, projected into the readerly void. As for Burgess himself, he probably had nothing more specific in mind than the narrowness he elsewhere complains of in English novelists, whose interests remain typically domestic and suburban when they could be "expanding the 145 146Philosophy and Literature resources of literature to find out more about die whole human complex, the roots on which societies are built."3 Suppose we take the parallel with Tolstoy seriously. War and Peace reads as if told in a voice which is that of the implicit author, a distinctive and cranky character represented for us by the name "Tolstoy" on the title page. A distinctive character, but not a character in the book; it is just the putative owner of a voice projected by the tone in which the book is written. However, much of the special effect of War and Peace comes from our tendency to identify this highly personalized voice-bearer with a real person. The authorial voice ofthe book becomes that ofthe person who wrote it, Count L. Tolstoy himself, quite widiout regard to who this person was in real life and what the real life was that he was in. The book reads like a communication, and mat is how many ofus read it. As it happens, the actual person Tolstoy was himselfat least sometimes prepared to read books that way. Our first question to a new author, he said when writing about Maupassant, is: "What sort of a man axe you? Wherein are you different from all die people I know, and what can you tell me that is new, about how we must look at this life ofours?"4 That is, a book is not only a text to exercise our readership on, and a story told for us to follow in delight or suspense, but also a message from a real person who writes, to real people who read, about the world they all share. Examination of a limited number of publisher’s Instructions for Authors, guidelines from two scientific societies, and the widely accepted policy document of the International Committee of Medical Journal Editors (ICMJE) provided useful information on authorship practices. Three of five journals examined (Nature, Science, and the Proceedings of the National Academy of Sciences) publish papers across a variety of disciplines. One is broadly focused on topics in medical research (New England Journal of Medicine) and one publishes research reports in a single (...) discipline (Journal of Bacteriology). Similar elements of publication policy and accepted practices were found across the policies of these journals articulated in their Instructions for Authors. A number of these same elements were found in the professional society guidelines of the Society for Neuroscience and the American Chemical Society, as well as the ICMJE Uniform Requirements for Manuscripts Submitted to Biomedical Journals. Taken together, these sources provide the basis for articulating best practices in authorship in scientific research. Emerging from this material is a definition of authorship, as well as policy statements on duplicative publication, conflict of interest disclosure, electronic access, data sharing, digital image integrity, and research requiring subjects’ protection, including prior registration of clinical trials. These common elements provide a foundation for teaching about scientific authorship and publication practices across biomedical and life sciences disciplines. (shrink)

The right to buy and sell human organs is challenged by the authors within the framework of a broad Christian perspective. Opposition to organ sales is argued in the light of the developing underclass of poor organ donors throughout the world who sell their organs to the rich. Very often neither the donors nor the recipients are fully informed about the medical risks involved in the procedure of organ transplantation.

Post-modernist thought represents the latest skeptical turn in a revolution going back to the overthrow of speculative thought in and after Hegel's time, whose principal phases are traced from its dogmatic origins in 19c scientism and absolutism, through the 20c. schools of meta-philosophy, to the explicitly post-philosophical positions of Derrida, Rorty and others who would finally abandon or suspend all engagement with the tradition of philosophical reason. The progress toward this denouement has brought with it progressive distortion of the understanding (...) of classical philosophical arguments on their own terms, an understanding which now needs to be recovered from a standpoint that takes account of the legacy of the ultra-philosophical critique but now knows it as itself limited. The words in children's language learning environments are strongly predictive of cognitive development and school achievement. But how do we measure language environments and do so at the scale of the many words that children hear day in, day out? The quantity and quality of words in a child's input are typically measured in terms of total amount of talk and the lexical diversity in that talk. There are disagreements in the literature whether amount or diversity is the more critical (...) measure of the input. Here we analyze the properties of a large corpus of speech to children and simulate learning environments that differ in amount of talk per unit time, lexical diversity, and the contexts of talk. The central conclusion is that what researchers need to theoretically understand, measure, and change is not the total amount of words, or the diversity of words, but the function that relates total words to the diversity of words, and how that function changes across different contexts of talk. With its over 40 000 denominations worldwide, Christianity undoubtedly remains the most fragmented of the religions of the world. One of the main causes of the said fragmentation is apparently the practice of tithing, which both genuine clergy and many shady characters that have disguised themselves as ministers of religion in society regard as the quickest way of accumulating wealth or making money. Anybody who views television programmes on religion and listens to religious leaders who give Christian preaching on various (...) radio stations nowadays cannot fail to observe the aggressive way in which such leaders opportunistically manipulate their followers and listeners to pay tithes using what has come to be generically known as ‘prosperity gospel’. Given the extent to which it has fragmented and continues to fragment the Christian Church, as well as to taint the image of Christianity as a religion, the question asked by many people today about tithing is: Are the Christians as obligated to pay tithes as the Jews under the Mosaic Law? Using typology for its interpretative tool and arguing both scripturally and historically, this article argues that Christians are not obligated to pay tithes because tithing, as part of the temple worship system whose existence ended with Christ’s free self-offering as a sacrifice to God on the cross, foreshadowed free offering to God by Christ’s followers, not obligatory giving by law.Contribution: This article contributes to the general debate on tithing as practised in the Christian Church today. The article argues that there is no scriptural proof anywhere to validate payment of tithes to the New Testament Church or its ministers. (shrink)

The increasing gap between organ supply and demand has opened the door for illegal organ sale, trafficking of human organs, tissues and cells, as well as transplant tourism. Currently, underprivileged and vulnerable populations in resource-poor countries are a major source of organs for rich patient-tourists who can afford to purchase organs at home or abroad. This paper presents a summary of international initiatives, such as World Health Organization’s Principle Guidelines, The Declaration of Istanbul, Asian Task Force Recommendations, as well as (...) UNESCO’s and the United Nation’s initiatives against trafficking of human organs, tissues, cells, and transplant tourism. Beyond the summary, it calls for more practical measures to be taken to implement the existing guidelines and recommendations, in order to prevent exploitation of the poor as organ providers. The paper suggests that an international legally binding agreement in criminalizing organ trafficking would be a step forward to bring a change in the global picture of organ trafficking and transplant tourism. We consider the moral and social ingredients in physicians' relationships with patients of diminished capacity by considering certain claims made about friendship and the physician's role. To assess these claims we look at the life context of two patients as elaborated examples provided in two novels: Woman on the Edge of Time (1976) by Marge Piercy, a radical feminist; and It's Hard to Leave While the Music's Playing (1977) by I. S. Cooper, a prominent physician-researcher. At issue is how the (...) doctor-patient relationship should be structured. In question is whether the physician's friendship and professional expertise, together with the diminished capacity of the patient, authorize medical paternalism. From our examination, we find compelling insights against appealing to friendship both in good doctor-patient relationships and in more typical, not-so-good ones. (shrink)

Policies and processes dealing with institutional conflict of interest lag well behind those dealing with individual COI. To remediate this, academic institutions must develop strategies for addressing some of the unique challenges in iCOI, including: clarifying the definition of iCOI that addresses the range of individuals potentially involved; implementing a well-designed electronic database for reporting and managing iCOI across multiple leadership constituencies; and providing ongoing education to appropriate institutional officials that communicates the importance of managing iCOI.

Background: Discussions about medical errors facilitate professional learning for physicians and may provide emotional support after an error, but little is known about physicians’ attitudes and practices regarding error discussions with colleagues.Methods: Survey of faculty and resident physicians in generalist specialties in Midwest, Mid-Atlantic and Northeast regions of the US to investigate attitudes and practices regarding error discussions, likelihood of discussing hypothetical errors, experience role-modelling error discussions and demographic variables.Results: Responses were received from 338 participants . In all, 73% of (...) respondents indicated they usually discuss their mistakes with colleagues, 70% believed discussing mistakes strengthens professional relationships and 89% knew at least one colleague who would be a supportive listener. Motivations for error discussions included wanting to learn whether a colleague would have made the same decision , wanting colleagues to learn from the mistake and wanting to receive support . Given hypothetical scenarios, most respondents indicated they would likely discuss an error resulting in no harm , minor harm or major harm . Fifty-seven percent of physicians had tried to serve as a role model by discussing an error and role-modelling was more likely among those who had previously observed an error discussion .Conclusions: Most generalist physicians in teaching hospitals report that they usually discuss their errors with colleagues, and more than half have tried to role-model discussions. However, a significant number of these physicians report that they do not usually discuss their errors and some do not know colleagues who would be supportive listeners. (shrink)

BackgroundConcurrent with efforts to establish national and regional biorepositories in Africa is widespread endorsement of ethics committees as stewards of the interests of individual donors and their communities. To date, ethics training programs for IRB members in Botswana have focused on ethical principles and international guidelines rather than on the ethical dimensions of specific medical technologies and research methodologies. Little is known about the knowledge and concerns of current and prospective IRB members in Botswana with respect to export, reuse, storage, (...) and benefit-sharing of biospecimens.MethodsThis qualitative study examined perspectives of IRB members in Botswana about the collection and use of biospecimens in research. Forty-one IRB members representing five committees in Botswana participated in discussions groups in March 2013. Transcriptions of audiotapes and field notes were analyzed to identify issues of concern that might be alleviated through education and capacity-building, and areas that required ongoing discussion or additional regulatory guidance.ResultsAreas of concern included lack of understanding among patients and providers about the use of biospecimens in clinical care and research; reuse of biospecimens, particularly issues of consent, ownership and decision-making; export of specimens and loss of control over reuse and potential benefits; and felt need for regulatory guidance and IRB-member training. Local belief systems about bodily integrity and strong national identity in the construct of benefits may be at odds with initiatives that involve foreign biorepositories or consider such collections to be global public goods.ConclusionEducation is needed to strengthen IRB-member capacity to review and monitor protocols calling for the collection and use of biospecimens, guided by clear national policy on priority-setting, partnerships, review, and oversight. Engagement with local stakeholders is needed to harmonize fundamentally different ways of understanding the human body and community identity with the aims of contemporary biomedicine. (shrink)
